So when a therapist/ counselor/ etc. says that everything is confidential except if you are a danger to yourself or others, who do they tell? And what constitutes a danger to self? Like does this include SH or my generally clumsy nature (a source of many concussions)? Like I never gonna tell a therapist Iām gonna kill myself but how much can I tell them before I get hospitalized?

If you show you're a danger to yourself your therapist can put you on 72 hour hold. They'd tell maybe hospitals most likely. Immediate family maybe. My own Dr came near committing me telling her I had suicidal thoughts. But it's ok to tell them that's what they're there for. It's to help you not to worry what they may say or do.
Under their confidentiality they're not supposed to tell anybody anything outside unless you show a danger then they may tell hospitals about you I'm not sure unless some can get your records and they document it. I'm not an expert though. Hope that helped
