So I’m in college and I haven’t talked to one of my friends for like a month or a few weeks even though we’ve known each other for like 2 years now and mutual friends have been expressing concern saying he’s a mess and emotionally unstable and his grades are slipping badly. I’m worried about him and wondered how I could maybe message him without sounding judgmental or condescending, as I’m struggling myself as well. Please help me figure this out!

This can be a pretty straight-forward message to your friend: "Hi...I can't believe how time has gotten away from me and we haven't talked in weeks. I could use a good talk. I miss you and hope things are going OK for you. Give me a call. I look forward to catching up."
Short and sweet is just fine. Sounds like you're finding yourself in a situation where you feel awkward about reaching out to your friend. Guess what? Your friend may be feeling the same way so just reach out to him.
You also mention struggling yourself. Make sure while you're worrying about your friend that you don't forget to take care of yourself. Hang in there.
